Following the conclusion of your children's curriculum classes, please complete the Children's Program Follow Up Report, directions are found here. 

The report will be completed online at the Madison Diocesan Data Hub.Click here to log into the Data Hub. 

The follow up report should be completed in a timely manner to assist the Office of Safe Environment in completing it's yearly audit requirements.  Please remember to include and count any students that were absent as "Opt-Out without form".   Following your report, please file your Opt-Out documents in a record keeping place, to reference if you need to in the future.  (please do NOT send them to the Office of Safe Environment)  If you need a reporting tool to aid you in collecting this information prior to reporting it on the data hub, please click the link below.
